Summary: St. Anne Hospital is looking for a compassionate Hematology/Oncology locum to provide high quality, patient centered care for many types of cancer and blood disorders for a community based ambulatory clinic in Burin WA. Details below.
Rates for submission(strictly follow)
Malpractice Must be included in rates.
Orientation ***-307.50- half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly ***-615 - Per hour, not OT
Vacation ***-922.50 - Must be 1.5x the hourly rate*** Per vacation hour, no OT
Schedule/Availability Requirements:
8a-5pm - Monday, Tuesday & Thursday - Site is flexible on the 3 days, please list the 3 days your provider can cover
12-15 shifts per month
