Rates to submit, *Strictly comply*

Malpractice
Must be included in rates.

Orientation
***-275 - half the hourly/callback rate*** kicks in after 8 hours. Per hour of
orientation or training beyond 8 hours

Hourly ***-*** Per hour, not OT

Vacation ***-825 - Must be 1.5x the hourly rate*** Per vacation hour, no OT

Nightly Call
***-*** - Per night call to hold the pager, to include 2.5 hours gratis

Vacation Nightly Call - ***-1800 - "Per night call to hold the pager, to include 2.5 hours
gratis

On Call (Daily) ***-*** - Per 24 hours to include 4 hours gratis"
