Job Scope:Community Care Occupational Therapists are responsible for the assessment and management of patients referred for occupational therapy (OT) by the Community Care Team and Community Health Posts. They will also participate in multi-disciplinary meetings as needed to provide consultative services and identify patients requiring OT input.
1) Clinical Duties- Review the suitability of referrals to the Community Care Occupational Therapist within set criteria
- Review and discuss cases which are escalated by nurses, doctors and allied health.
- Screening and referral of potential cases (discharged from Inpatient) to Community Care Team (CCT)
- Conduct home visits, telehealth consults, SOC and Community Health Post (CHP) visits to assess and manage OT related issues. ...
