Role Overview
The Manager, Future Care Planning (FCP) will report to the Senior Manager, FCP, which is under the umbrella of Community-Based Support Services (CSS). CSS is the dynamic and nimble service delivery arm in MINDS that respond to emerging needs of PWIDs and Caregivers in the community.
Responsibilities- Assist the Senior Manager to oversee programmes, which includes Life+, Life Coaching and Home-based Care services.
- Life+ is a pilot initiative to enable and sustain inclusive community living and ageing in place for PWID through tailored client support, future care planning and the building of community capacities.
- Manage risk and quality of the programme and play a pivotal role to ensure the smooth execution of the programmes and assist in manpower management and allocation.
