Job Description
The Population Health Medical Assistant supports primary care teams within the network, either remotely or from a primary care office, to enhance compliance with population health initiatives such as annual wellness visits, quality care gap closure, and in-network referrals. The role does not involve direct patient care but involves executing standing orders for patients, scheduling appointments and closing care gaps.
Responsibilities
+ Regularly review patient charts for screening needs, quality gap closure, and care coordination requirements.
+ Utilize standing orders developed by primary care physicians to place orders for screening and quality gap closure needs, working under the appropriate physician licensure.
