**Overview**
**7 on / 7 off Shift: 10:30a- 9:00p**
**Description**
+ The Medication Reconciliation Assistant (MRA) practices within the health system and works under the supervision of a pharmacist to collect and document the best possible medication list, including the correct drug name, strength, route, and administration frequency for all current prescription and over-the-counter medications.
+ The MRA will interview patients, their families, external medical providers/facilities, and/or outpatient pharmacies to confirm the accuracy of the medication list.
+ The MRA coordinates corrections to a patient's medication list by notifying physicians and/or pharmacists of updated information.
+ The MRA may obtain, add, or correct medication allergy information including the medication name and type of reaction.
