Overview
Barrow Home Based Treatment Team (HBTT) within Lancashire and South Cumbria NHS Foundation Trust (LSCFT) is seeking a Mental Health Practitioner to provide comprehensive assessment and triage services in the community.
The HBTT works with individuals in acute mental health distress to avoid hospital admission and support early discharge.
Key Responsibilities
- Provide comprehensive assessment and triage service, signposting or facilitating onward to appropriate mental health pathways.
- Receive referrals from GPs, police, self-referrals, initial response service, acute hospital colleagues and other LSCFT agencies.
- Collaboratively triage service users and make decisions in consultation with referrers.
- Provide evidence-based mental health assessments and signpost to alternative service provision for urgent needs.
