**Job Summary and Responsibilities**
As our Nurse Navigator at Good Samaritan Hospital, you will serve as the first point of contact for patients and their families/caregivers. You will work directly with them and their providers to eliminate barriers to timely care, facilitate flow through the system, and facilitate interactions with the clinical care team.
+ Educate patients on preoperative, intraoperative, and post-operative experiences.
+ Facilitate communication between all members of the health care team.
+ Coordinate office visits across comprehensive care teams for continuity of care delivery.
+ Enhance each patient's experience by staying involved- visiting patients at appointments, assessing their needs, and relaying relevant information to providers.
+ Collect data for best practice and quality of care.
