Position Overview
PACT is an existing program that provides wrap‑around interdisciplinary services to individuals with severe and enduring mental illness. The community PACT teams work with individuals who have a primary psychotic disorder (Schizophrenia, Schizoaffective, Bipolar I) and may also have co‑existing medical conditions, addictions, or involvement with the criminal justice system.
Responsibilities
- Provide case management services and coordinate community supports for participants.
- Serve as liaison point for program participants, linking community partners and including families and natural supports as indicated in care plans.
- Coordinate medical services and medication management for individuals receiving PACT services.
- Provide crisis on‑call services and administer oral medications and long‑acting injections as prescribed.
