SUMMARY
The Nurse Navigator for Classical Hematology plays a vital role in improving referral efficiency, patient access, and care coordination for an expanding, nationally recognized tertiary referral clinic. This position supports appropriate triage of referrals, manages clinical triage communication when needed, streamlines complex care processes, and promotes consistent, high-quality patient care across the Division of Hematology and Oncology, in alignment with the stature and reputation of UT Southwestern.
This job will assist the patient, family and care giver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.) Responsible for conducting on-going assessments of need and facilitate referrals throughout the nursing continuum of care.
