Job Description
The Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.
Responsibilities :
+ Involve with direct patient physical assessment, education, and health planning
+ Maintain continuity of care by regular patient follow-up
+ Assist in the coordination of services with other disciplines and patients' insurance plans
+ Advocate for patient in arrangement of medically necessary services
+ Communicate with community resources to ascertain adequate assistance for patients in need
