Job Title: Nurse Practitioner / Physician Assistant (Primary Care)
Location: Phoenix, AZ
Hours & Schedule: Monday – Friday | 7:30 AM – 4:30 PM
Work Environment: Outpatient private practice with three established locations
Salary / Hourly Rate: $120,000 – $150,000 annually
Benefits Offered: Medical, Dental, Vision, 401(k), 4 weeks PTO (includes sick time, vacation and CME); after two years, PTO increases to 5 weeks
