About the Role
- Provide high‑quality nursing care to patients in the community.
- Conduct comprehensive assessments and develop individualized care plans for patients with wound, ostomy, and continence concerns.
- Provide patient‑focused, safe, holistic nursing care and work collaboratively with team members.
- Ensure nursing care is based on current best practices to achieve successful outcomes.
- Continuously assess, anticipate, and respond to new and changing patient needs.
- Travel between patients’ homes within a specific geographical region.
Qualifications
- Minimum of two years of nursing experience (community nursing experience is ideal, but not required).
- Completed NSWOC or International Interprofessional Wound Care Course (IIWCC).
