Job Title:  Occupational Therapist – ACE Team
Location: Rhondda Cynon Taf (Hybrid – Community & Remote)
Pay Rate: £31.46/hour
Working Hours: Full-time, 37 hours/week
Contract Type: Locum full time

What will your responsibilities be?


Accessing Services

  • Provide advice, information, and signposting

  • Assess urgency of referrals and initiate enquiries

  • Safeguard vulnerable individuals and ensure accurate record-keeping

  • Transfer information to partner agencies in a timely manner

    • Assessment of Need

  • Conduct holistic, person-centred assessments

  • Collaborate with individuals and carers to agree personal outcomes

  • Explore creative solutions to maximise independence and choice

  • Arrange equipment, adaptations, and inter-agency support
