Job Description
Our client is a community hospital providing integrated rehabilitative and long-term care services for patients recovering from illness, surgery, and age-related conditions.
Responsibilities- Assess patients’ physical, cognitive, and functional abilities to determine rehabilitation needs
- Develop and implement individualized therapy plans to improve daily living skills
- Conduct therapy sessions focused on mobility, self-care, and functional independence
- Train patients in the use of assistive devices and adaptive techniques
- Collaborate with doctors, nurses, and multidisciplinary teams on patient care plans
- Educate patients and caregivers on home care strategies and safety practices
- Monitor patient progress and adjust treatment plans accordingly
- Maintain accurate documentation and ensure compliance with clinical standards
- Degree in Occupational Therapy
