Occupational Therapist - Home Based Primary Care

Summary This Occupational Therapist position is assigned to Home Based Primary Care. This position will be located in St. James, MO until our new facility in Rolla, MO becomes operational. Responsibilities This assignment carries primary responsibility for providing in-home occupational therapy assessments, and the planning, administration and management of rehabilitation in the home for patients of the Home Based Primary Care Program (HBPC). The OT will provide these in-­home services to patients with a wide variety of multiple, complex chronic illnesses and/or profound disabilities requiring knowledge of contemporary and evidence based Occupational Therapy practices. Provides professional independent services for both assessments and development of patient treatment plans.
