About the Role:
Join the dynamic and fast-paced Integrated Discharge Service (IDS) team within Islington Social Services. IDS supports residents of Islington being discharged from hospital over 7 days a week. The Occupational Therapy role is crucial in assessing risk and designing interventions at various stages of the hospital pathway to support individuals in using their strengths and achieving their outcomes.
What will your responsibilities be?
In this role, you will support individuals on the Discharge to Assess Pathway by screening referral documentation and planning safe discharges home.
