Job Description: Conduct comprehensive assessments and deliver tailored therapy interventions based on individual needs. Develop personalised goals and treatment plans aligned with clients' daily living needs and life contexts. Provide therapy through individual or group sessions in centre-based, residential, or community settings. Train and guide caregivers to enhance clients' independence in daily activities. Work closely with healthcare professionals and participate in case discussions for holistic care planning.
