Responsibilities:


  • Conduct structured intake assessments capturing relevant information including service history, physical ability, trauma exposure, substance use history, suicidal ideation, mental health history, stabilization needs, and other pertinent information in accordance with EOVS documentation standards.

  • Assist veterans in identifying immediate needs, stabilization priorities, and short-term goals.

  • Support the development of Individual Recovery Plans (IRPs) or comparable goal-oriented service plans focused on stability, rehabilitation, community reintegration, and reduction of systemic barriers.
