The Bellfield Centre is looking for an experienced, dynamic and dedicated Intermediate Care Worker to join our vibrant and supportive team. This is an exciting opportunity for someone with a background in social care who is passionate about making a real and lasting difference to the lives of older people using our integrated Intermediate Care service. As an Intermediate Care Assistant, you will play a key role in supporting older people to regain independence, improve wellbeing and achieve their personal outcomes. You will work as part of a fully integrated health and social care team, providing high quality, person centred care and support.
