Reporting to the Director of Patient Flow, the Patient Flow Coordinator plays a key role between Providence Care's staff, physicians and external partners in ensuring seamless, quality and efficient patient flow activities. The Coordinator responds to inquiries from key stakeholders directly and/or indirectly and assists the Director by researching and providing requested data or background information, as requested. The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care & Transitions Specialists. The Coordinator completes daily, weekly and monthly summaries provided to relevant 'leaders in the hospital and is fully informed of issues and challenges that require action. Utilizing s...
