Per Diem rates:
In-person initial visit $180
In-person follow-up visit $100
Telehealth follow-up visit $70
Per diem coverage: The New Jewish Home in Manhattan and for community cases: Manhattan and Brooklyn. These geographies may change over time as census changes and as staff may be reassigned to different territories.
