Responsibilities
- Deliver specialised pharmacotherapy counselling with an emphasis on chronic disease management, medication safety, and enhancing patient quality of life.
- Review and verify prescriptions to ensure legal compliance, completeness, accuracy, and clinical suitability.
- Collaborate with prescribers to address medication‑related concerns and escalate complex issues when appropriate.
- Oversee dispensing, labelling, repackaging, and compounding activities in accordance with regulatory and organisational requirements.
- Conduct medication reconciliation during care transitions, identify discrepancies, and ensure appropriate documentation and resolution.
- Detect, prevent, and manage medication‑related problems through comprehensive medication reviews and optimisation of treatment plans.
