We are seeking a Physical Therapist to provide rehabilitation services to patients in a Skilled Nursing Facility (SNF) and Long-Term Care setting. The ideal candidate will be passionate about helping patients improve mobility, function, and overall quality of life.
Key Responsibilities
- Evaluate and develop individualized treatment plans
- Provide physical therapy services to residents in a skilled nursing and long-term care environment
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with physicians, nursing staff, and interdisciplinary teams
- Maintain accurate and timely documentation
Requirements
- Active Physical Therapist License
- Experience in Skilled Nursing Facility (SNF) or Long-Term Care preferred
