About the role
You will practice within a high-touch, team-based model to oversee the comprehensive health and well-being of a select patient panel. Working alongside a Registered Nurse and a Patient Care Administrator, you will participate in regular team huddles, coordinate proactive care plans, and deliver personalized, relationship-driven medicine.
Key responsibilities
Establish and maintain long-term, trust-based relationships with a curated patient panel.
Manage chronic and acute illness, preventive care, and episodic injuries.
Conduct well checks and occasional in-home visits when clinically appropriate.
