Job Description
The Eddy Senior Care, a member of St. Peter's Health Partners, is an established and respected PACE program (Program of All Inclusive Care for the Elderly) that is seeking a full time Internal Medicine/Geriatric Physician to join its team in 2016. This is an excellent opportunity to join a unique practice environment which provides physicians the opportunity to work with an interdisciplinary team to support frail seniors who remain living in the community and thus avoid nursing home placement. It is a program driven by relationships- not the number of encounters. The physician follows the participant whether at home, in the medical office, or in selected nursing homes should placement be needed.
