Physician - Primary Care

Summary This position is located in the Primary Care Service, at the Eastern Colorado Health Care System and provides support to the primary care of Veterans and their significant others within the ECHCS catchment area. Our Primary Care Provider functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model). The PACT team model consists of an MD (or PA/NP), RN, LPN, and Medical Support Assistant.
