Our client in the Non-Profit/Health Care Sector is currently seeking a Placement Coordinator to join their team on a contract basis starting on a 3 month contract. This role will be onsite in Westboro, with hours Monday-Friday 7am-3:30pm.The successful candidate will come with previous experience working in the medical field.
Responsibilities:
• Collaborate with Champlain LHIN (Local Health integration Network) to maximize full occupancy and attend meetings, as needed
• Review all resident applications for admission to determine appropriate level of care; assist with processing of admission and discharge papers
• Maintain the waitlist and internal transfer list
• Coordinate internal transfers including the update of documentation
• Process paperwork for deceased residents
