As a Primary Care Physician with CenterWell, you'll work onsite at our Powdersville clinic and report to the Lead Physician. You'll join a supportive, team‑based environment alongside nurse practitioners, pharmacists, care coaches, behavioral health specialists, and others—all collaborating to deliver personalized, high‑quality care.
**Main Responsibilities**
+ Provide high‑quality primary care aligned with clinical and value‑based standards.
+ Collaborate with the integrated care team to enhance patient care.
+ Manage referrals, care transitions, DME, and home health services.
+ Document care and ensure regulatory compliance.
+ Support daily huddles, on‑call rotations, and center programs.
**Qualifications**
+ MD or DO from an accredited program.
+ Board Certified or Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
