Job Description
Responsibilities
- Facilitate seamless integration of quality patient care services
- Conduct patient education at various intervals
- Coordinate and monitor patient visits from participation through completion
- Track and follow up on patient visits according to project protocol
- Maintain patient confidentiality throughout the process
- Lead day-to-day project operations and coordinate activities across departments according to established guidelines and service agreements
- Act as primary liaison between stakeholders, agencies, and project teams
- Organize and facilitate project meetings, including kick-off sessions and progress updates
- Track project milestones, deliverables, and timelines as specified in contracts
