Job Purpose
The Recovery Coordinator plays a key role in supporting individuals affected by substance misuse and related issues to achieve sustained recovery. You will provide person-centred interventions, coordinate care plans, and work collaboratively with partner agencies to improve health, wellbeing, and social outcomes.
Key Responsibilities
1. Case Management & Support
Manage a caseload of service users with varying needs (e.g. substance misuse, mental health, housing, offending history).
Complete comprehensive assessments and develop tailored recovery care plans.
