Referral Co-ordinator

We are recruiting for a Referral Coordinator to join our Single Point of Access (SPOA) team on the Isle of Wight.

You will conduct comprehensive patient assessments, provide expert advice, and direct patients to appropriate resources as a key member of the Community Single Point of Access (SPOA) team.

Your role will be to serve as a referral coordinator, executing referral reviews and telephone triage within a collaborative, inter-professional team.

Collaborate with healthcare professionals to deliver high-quality services to clients across a diverse range of community services.

Engage in an integrated service model, delivering multifaceted interventions. Please note that this role may necessitate weekend and bank holiday work on a rotational basis in the future.
