About the role:
The successful applicant will be responsible for:
- Serve as the point of contact, advocate, and informational resource for the Community Care Services team, patients, families, and community resources.
- Facilitate and attend meetings between patients, families, care teams, and community resources as needed.
- Lead, develop, and support a client-focused team of staff and volunteers who manage the provision of support services to clients.
- Ensure the delivery of high-quality services by staff that meets all relevant team performance and outcome indicators.
