DUTIES:
Assess / monitor the residents' nutritional status, provide recommendations to medical staff, and develop adequate care plans
Collaborate with residents and interdisciplinary team members to obtain and maintain food preferences consistent with clinical nutrition needs, ensuring accurate entry and ongoing updates within the foodservice software system.
Evaluate special needs of residents regarding nutrition support, skin breakdown, and significant weight issues
