Summary The Caregiver Support Program Clinical Assessor, Registered Nurse, is responsible for knowledge of eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day-to-day operation of the Caregiver Support Program (CSP), and all reporting requirements within the assigned Veterans Integrated Systems Network (VISN). Responsibilities The Caregiver Support Program Clinical Assessor, Registered Nurse, Critical Care, duties include, but are not limited to the following: Assesses, plans, implements and evaluates care based on age-specific components. Delivers fundamental knowledge-based care and counseling to assigned clients while developing technical skills. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
