Experience:
- 5 years of RN Case Management experience
- Inpatient Acute Case Management (Hospital experience required)
Job Duties:
As a Case Manager RN, you will be responsible for coordinating care and planning transitions throughout the acute care patient experience. Collaborating with physicians, utilization managers, medical social workers, and bedside RNs, you will ensure the timely progression and transition of patients to the appropriate level of care, preventing unnecessary admissions or readmissions.
