The Community Care Coordinator is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. Located at the Hruska Federal Courthouse in Omaha, NE. Responsibilities This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 1, 2026 or all vacant positions are filled.
