Overview
The Registered Nurse (RN) Case Manager collaborates, coordinates, and delivers healthcare services with advocacy and a patient- and family-centered approach. You will work with clients across the life span in the home setting. You must be willing to drive up to one hour from your home site to clients’ residences during all seasons and at various times of the day. This position has on-call duties and certain physical requirements such as bending, lifting, twisting, walking, climbing stairs, and carrying. The lifting and carrying could include heavy items.
