Summary THIS IS NOT A REMOTE POSITION AND WILL BE LOCATED AT THE MILWAUKEE VAMC. The GEC Home Telehealth RN Care Coordinator executes position responsibilities that demonstrate leadership and creative approaches to the management of complex patient care. Care will be provided in the least restrictive, most cost-effective manner. HT is a program that applies care and case management principles to coordinate care using informatics, disease management, protocols, and individualized nursing interventions. Major duties and responsibilities: Provide initial and ongoing comprehensive assessments to include a review of systems which establishes a comprehensive plan of care. Lead and organize delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice.
