Summary The Registered Nurse Housing and Urban Development - Veterans Affairs Supportive Housing (HUDVA) is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Responsibilities The HUDVASH RN develops a plan of care using information from the patient, family, and provider. Bridges the gap between discharge and primary care follow-up. Identifies high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care etc.). Responsible for intensive monitoring, follow-up, and clinical management of high-risk patients. Major duties and responsibilities: Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice.
