Job Description
We are hiring Registered Nurses (RN) or Licensed Practical Nurses (LPN) clinical experience in acute care, ambulatory care, home health, or care management.
Key Responsibilities
- Conduct telephonic and virtual patient assessments
- Educate patients on conditions, medications, and treatment plans
- Develop and manage individualized care plans
- Monitor patient progress and escalate clinical concerns when required
- Coordinate care among providers, payers, and community resources
- Maintain accurate documentation in EHR systems
- Support patient engagement and continuity of care
Required Skill
• Strong clinical assessment and patient communication skills.
• Knowledge of chronic disease management and preventive care.
• Experience with EHR/EMR systems and telehealth platforms.
• Ability to work independently in a remote care environmet
