Summary The Registered Nurse - Patient Aligned Care Team (PACT) Care Manager delivers and improves holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned. Responsibilities The Registered Nurse (RN) - Patient Aligned Care Team (PACT) Care Manager works in partnership with an assigned panel of patients and team members for effective disease and chronic care management. They use the nursing process and evidence-based practice to collaborate with the expanded team, internal and community-based services, in developing the patient driven care plan for prevention and management of diseases. Major Duties include, but are not limited to: Responsible for collection of data and implementing evidence based changes to practice based on data results.
