Registered Nurse-Patient Aligned Care Team/Homeless PACT Outpatient Staff

Summary You will provide care coordination for veterans who are assigned to the Homeless Patient Aligned Care Team (HPACT) for the Balcones Heights Primary Care Clinic located in the Wonderland of the Americas Mall. You will provide comprehensive, coordinated nursing care to Veterans experiencing or at risk for homelessness as part of an interdisciplinary PACT. Responsibilities You will work with Veterans and their families, collaborating with medical providers, social workers, and case managers to deliver patient-centered care in the outpatient setting. Provide care coordination for veterans who are assigned to a Patient Aligned Care Team (PACT) for the Balcones Heights Primary Care Clinic located in the Wonderland of the Americas Mall.
