What You’ll Do
- Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program.
- Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies.
- Early Intervention: Monitor symptoms, identify risks, and elevate care before complications arise.
