The case manager is responsible for directing the care of clients under the Home Care program: you will use strong assessment, organizational, and leadership skills to identify client needs and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching. The care plan will be developed, reviewed, and modified on an ongoing basis.
