Summary The Traveling Veteran Coordinator Registered Nurse position is aligned with Primary Care service line at the Chillicothe VA Medical Center, Chillicothe, Ohio. Traveling Veteran Coordinator Registered Nurse is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. Responsibilities Duties and responsibilities include but are not limited to: Assists with facilitation of physician/clinical consultations, as needed, to render timely clinical decisions to expedite care. Serves as the point of contact for transfers out to higher levels of care and coordinates transportation as needed. Responsible for tracking data related to transfers out and admissions to acute mental health. Assumes responsibility for the coordination of care for traveling Veterans either vested in another Veterans Affairs (VA) facility and traveling to the local catchment area, or traveling outside of the local catchment area, focused on patient education, s...
