Overview
Seasons (a certified Great Place to Work®) is looking for a Care Coordinator (RPN) to supervise and provide high‑standard care services. This role maintains a safe environment for residents, families and staff while mentoring Resident Guest Attendants.
What You Will Do
- Effectively utilize the nursing process to provide high quality nursing care.
- Perform supervisory duties, ensuring excellent care and mentoring the care team.
- Conduct resident assessments, manage transitions, and perform 6‑month re‑assessments per RHRA regulations.
- Establish priorities based on individual resident needs.
- Develop person‑centered service plans.
- Ensure compliance with Resident Assessments/Service Plans (PCC) and community policies.
- Manage risk through monthly care‑related audits.
- Administer medications (PCC knowledge a plus).
