RN Case Managers work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. Essential to this role are the core values of patient advocacy, holistic assessment, care planning and the empowerment of patients and families to self-manage.
**This is a PRN role. A caregiver in this position works on-site, 2 Mondays, 2 Fridays and 1 weekend shift per month. Floating across Cleveland Clinic Main Campus is required.**
