RN Case Manager - Position Highlights

  • Location: Northwest Houston, TX - Hybrid - Remote
    This is a hybrid, field based role requiring a dedicated home office and the ability to travel 3-4 days/week within an assigned territory. Coverage includes the Northwest Houston and surrounding areas. Territory assignments may change cased on business needs.

  • Schedule : Monday - Friday
    Flexibility is required to accommodate patient needs and nephrology partner availability.
