RN Clinical Hospice Liaison
Role Overview
A registered nurse–based liaison role focused on building relationships with hospitals, physicians, and healthcare facilities to increase hospice referrals and support patient access to hospice services. The role combines clinical expertise with outreach, education, and relationship development.
Schedule & Location
- Monday to Friday, 8:00 AM – 4:30 PM
- Mesa, Arizona (field-based hospital and facility outreach)
Key Responsibilities
- Develop and maintain relationships with physicians, hospitals, discharge planners, case managers, and care facilities
- Identify key decision-makers and strengthen referral networks
